Homeless Healthcare Hub: A Central Base for Comprehensive Care
Homelessness is not always a linear journey from the streets to independent housing—it is often cyclical. Our service model is built on the understanding that the single most impactful way to improve a homeless person's health is to provide them with safe and secure housing, while simultaneously stabilising their health issues.
A Central Hub for Outreach and Care
Located in Highgate, just 1km north of Royal Perth Hospital, the Homeless Healthcare Hub serves as:
1. The Base for Mobile Operations
Street Health – Outreach to rough sleepers, led by registered nurses (RNs) and caseworkers.
Mobile Clinics – Providing healthcare at drop-in centres, transitional accommodation services, domestic violence shelters, and drug and alcohol rehabilitation services.
Together Program – Outreach support for pregnant women experiencing homelessness and their babies.
2. The Administrative Centre
Coordinates all Homeless Healthcare services across the region.
3. A Fixed-Site Clinic for People with Lived Experience of Homelessness
Unlike a standard GP practice, this clinic is designed to meet the complex healthcare needs of its patients:
Longer consultation times to manage multiple health conditions.
Enhanced reception support to help patients maintain appointments.
Flexible appointment times, ensuring patients are accommodated if they arrive early or late.
Higher nursing ratios to provide intensive care for complex health needs.
Transport assistance, ensuring patients can access treatments unavailable on the street or at drop-in centres.
A dedicated women’s clinic, offering a safe and supportive environment for women, including victims of domestic abuse, who may feel uncomfortable in mixed-gender clinics.
4. A Base for Allied Health Services

Potential space for a future dental clinic, specifically catering to individuals with a history of homelessness.
A Hub-and-Spokes Model for Holistic Care
Homeless Healthcare operates on a hub-and-spokes model, allowing clinicians to provide care at all stages of a person’s journey—whether on the streets, in temporary accommodation, or transitioning to stable housing.
By working across different settings, our clinical staff build trust and rapport, ensuring patients receive continuous, coordinated care in an environment where they feel comfortable.
