Mental Health Street Outreach
Homeless Healthcare has recently commenced a Mental Health street outreach service in the Fremantle CBD.
The team provide outreach and mental health intervention twice a week for people with complex health needs that are experiencing chronic homelessness and rough sleeping in Fremantle. They have also secured a safe space in the council building to conduct mental health assessments.
The mental health clinician within the team has been joint funded by the State Government and the City of Fremantle and is a first for a WA local government. The funding will allow the service to operate for 2 years.
The team provides assertive outreach, engaging with rough sleepers and identifying those who require mental health support. The team works collaboratively with other health and mental health services and makes referrals where appropriate.
