Although homelessness can be a linear journey from the street to independent housing it is often cyclical.

Our service model is based on the premise that the single biggest thing that can be done to improve the health of a homeless person is to find them safe and secure housing. This will usually require stabilising of health issues at the same time.

Located in Highgate, 1km North of Royal Perth Hospital, the hub is:

    • The base for Homeless Healthcare’s mobile operations including

      • Street Health- Outreach to street present people led by RNs and caseworkers

      • Mobile clinics to drop-in centres, transitional accommodation services, domestic violence shelters and drug and alcohol rehabilitation services.

      • Home Visiting GP Service

      • After-hours Support Service

    • The administrative centre for all our services.

    • A fixed-site clinic for people with a current or past lived experience of homelessness that operates in a similar manner to a regular GP practice except

      • Consultation length is longer (to manage complex health needs of patients)

      • Patients are heavily supported by reception staff to maintain their appointments

      • Appointment times can be fluid and patients will be accommodated, within reason, should they arrive early or late

      • Nursing ratios are higher (due to complexity of health)

      • Patients can be transported to the clinic to receive care that cannot be administered on the street or in drop-in centres

    • A women’s clinic enabling victims of domestic abuse and others who find mixed clinics challenging to see GP in a safe and understanding environment.

    • A base for allied health services including

      • Physiotherapy

      • Podiatry

      • Counselling

    • Has potential space for a dental clinic catering for people with current or past histories of homelessness

The hub and spokes model of Homeless Healthcare’s services enables our clinicians to see people experiencing homelessness at all stages of their journey, in an environment where they feel comfortable.

As our clinical staff work across the services, seeing patients in a variety of settings, it helps them build trust and rapport and provide continuous care.
